Preliminary form for travelers to the Czech Republic

From 5 February 2021, the Czech Republic introduces a new system for determining the countries at risk of becoming 
infected with COVID-19 and new rules for the entry into its territory by passengers arriving from those countries.
Arriving from Bulgaria is required to fill in and send an electronic Public Health Passenger Locator Form in advance, 
as well as to present on arrival in the Czech Republic an antigenic test for COVID-19, not older than 24 hours, 
or a PCR test made no more than 72 hours before arrival. In addition, within 5 days of their arrival in the Czech Republic,
 they should re-perform a PCR test and submit it to the local hygiene and epidemiological service, 
by which time they must isolate themselves. 10 days after arrival, they are also required to wear a FFP2 standard surgical
or protective mask.

Exceptions to the above rules for international transport, for passengers in transit through the territory 
of the Czech Republic and for cross-border workers continue to apply.
The rule remains that trips to the Czech Republic are only possible in cases of extreme necessity 
and for a significant reason, and the already permitted stay for such trips is up to 12 hours.
